The basis of such dangerous attacks as myocardial infarction (Ml), stroke and sudden death is atherosclerosis and its risk factors — arterial hypertension (AHT), dyslipidemia (DLP), cigarette smoking and others. Besides, there are many other functional factors which may provoke fatal complications of atherosclerosis: physical effort, negative emotions, the rise of blood pressure, arterial spasm, etc. Sometimes functional rather than organic components become more important in the pathogenesis of MI, stroke and sudden death. The relations between organic and functional factors of fatal complications are analyzed in the book. These dangerous complications are usually quite unexpected but in most cases some latent symptoms are present which suggest that the patient’s life is menaced. The predictors for MI are ischemic heart disease (including silent forms) and unstable angina. For sudden death they are ectopic frequent ventricular premature contractions; the predictors for stroke are silent plaques in carotids, cerebrovascular encephalopathy, etc.

Special emphasize is placed on primary prophylaxis of MI, stroke and sudden death. Prevention of atherosclerosis as well as secondary prophylaxis are cessation of atherosclerosis progression and prevention of its dangerous complications is becoming a reality due to modern, effective drugs (statins, ACE inhibitors, anticoagulants, mononitrates, etc).
